Title: Program Officer (TAS Lead)

AIC Kijabe Hospital a church based hospital is seeking to fill the following position: Program Officer (TAS Lead)

Main Responsibilities
 
1.  Establishing, Promoting and sustaining linkages between the community and the clinic
  • Making sure that community reports including adherence issues are communicated appropriately and documented.
  • Form linkages between the LPTF, the community and other stakeholders that will be patient focused.
  • Help define geographical catchments area.
  • Identify CHWs to be trained together with the other community nurses, and conduct the trainings Help establish support groups together with other community staff.
  • Help identify and establish satellite clinics sites in the community.
  • Together with other staff help patients establish support groups.
2. Coordinating and supervising
  • Organize, coordinate and facilitate community activities. 
  • Coordinating community outreach and mobilization activities e.g. support groups, trainings, meetings, referrals and defaulters tracing, HIV testing and give feedback to the program coordinator.
  • Coordinate and supervise community staff working under him/her.
  • Ensure community staffs have the necessary requirement for work including finance.
  • Ensure accountability as per CHAK-CHAP budget and Kijabe Hospital policies.
  • Review and analyze adherence officers and social workers reports including their daily activity reports, enrollments and monthly activities in the region.
3. Home based Care, adherence support and counseling
  • Make sure adherence issues are followed up in the community.
  • Ensure priority home visits /patients follow ups are done and feedback given to the clinician.
  • Make sure support groups are well co-ordinate to enhance adherence.
  • Provide home based adherence support and counseling.
  • Refer patients to clinic for enrolment.
  • Conduct Home- based testing for family members.
  • Assess for O.Is in the homes, treat minor illnesses and refer difficult cases accordingly.
  • Look out for drug related side effect, manage or refer accordingly.
  • Ensure initial and follow up home visits are done by adherence officers.
4. Provide timely and quality regional progress reports as per donor requirements
  • CBTS report
  • PWP report
  • Monthly activity reports.
  • Others as requested by the donor or the program manager.
  • Teens psychosocial template
  • Any other report as deemed necessary by the program manager.
5. Resource management
  • Proper reconciliation of advanced funds within the agreed period of time.
  • Proper use of the motorbikes, community kits and other program resources.
  • Supervising the adherence officers and the social workers on proper utilization of resources.
Minimum acceptable qualifications & experience:
  • Diploma in nursing (KRCHN) or a degree in community development and social work with basic HIV training.
  • Dedicated and highly motivated professional.
  • Desire to see CHAK team fulfill their purpose.
  • Good interpersonal and teamwork skills.
  • Has previously worked in the community as a HIV community nurse or a social worker.
  • Three years experience of working within HIV-related field
  • Experience of teaching / mentorship.
